Patients with certain long-term conditions are offered an appointment with a nurse every year for a review. These reviews are to monitor your condition, help you manage your condition and ensure that you have the most appropriate support or treatment.
If you have any concerns about your condition at any other time, please contact reception for an appointment and do not wait for your annual review.
All patients with the following conditions will be offered an annual review appointment with the nurse:
- Asthma
- COPD
- Dementia
- Diabetes
- Epilepsy
- Heart Disease (CHD)
- Hypertension
- Hypothyroidism
- Learning disabilities
- Osteoporosis
- Rheumatoid Arthritis
- Severe Mental Health Illnesses
- Stroke
