This ensures:
• Your request is reviewed safely by a GP
• You are directed to the right service first time
• Our clinicians can prioritise patients who need urgent care
📌 If you cannot access the internet, ‘EMMA’ (our virtual receptionist), or a member of our Care Navigator team will help you complete the form over the phone.
