New Patients

The Registration Process

To register as a patient at Engleton House Surgery you can register online by filling out the form below, or by obtaining a form from reception.

If you already have one, you will need your NHS medical card or NHS number, together with personal identification. If you do not already have an NHS number then this will be provided once all the registration documents have been processed. New patients may request a health check with a practice nurse.

Practice Boundary

To register at the practice your home address must be within our practice boundary. You can check if you are eligible to register by using the map and postcode checker below. If your address is on the edge of our boundary we advise calling reception to confirm whether you are eligible:

Catchment Area

Please note that every care has been taken in the preparation of information on this Postcode Mapping Tool, however there may be instances where the Postcode Mapping Tool does not include your location even though it is within our Practice Boundary. An example of this may be new housing developments where the postcodes are not yet included on Google.

Temporary Patients

If you are registered at another GP surgery in the UK, but are staying temporarily in our Practice area and need to see a GP, you may register at the practice as a Temporary Resident. We can then provide you with the medical care you require for the duration of your stay.

To register as a temporary resident you will need to complete and sign a Temporary Resident Form. You can obtain this form from the surgery reception.

Overseas Patients

Please help us to keep your record up to date by informing us of any changes to your name, address, telephone numbers (including mobile) or email address. This can be done in person, in writing or online. We need to have the most up to date information so that we can contact you about anything urgent.
